State Machine Issue

I am trying to create state machine workflow. But whenever workflow reaches change state action, it stays there for some time and send an error email.

Had anyone faced this issue in the past ? Not sure what could be the reason for this workflow behaviour.

I have also tried giving some delay before changing state but still it throws error at change state action. 

 Need help on the same.
